Admission Application for Lighthouse
Complete the form below to initiate your admission to Lighthouse. If you are under 16 years of age, one of your parents must sign the form as well if you under age. This is only the first step of your enrollment. Once the form is received, and Admission Counselor will be assigned to you to finalize your enrollment as other pertinent information may be required to do so. $25.00 non refundable application fee is required for this form. A copy of your last transcript is required if left school in the last five years.
